Well, there's several design decisions that impact a lot the quality in an oil filter. Fram used to use paper on the top and bottom ends of the filter when everyone else used metal sheets. they are using metal sheets now. then there's valves designs that instead of going on the outside (and failing outside the filter) goes on top with the tensioner and if failure occurs, are jettisoned right into the engine (as fram used to have, a plastic valve no less). Then there's several other factors that impact oil flow but this comment will be too big.

As with everything in a car: everything can fail :) and usually not close to the manual replacement time. Just follow symptoms. if something is giving you a clue that the timing chain is extended or broken, go after it (after checking other easier things that may also cause the same symptom, of course)

no smoke on exhaust means there's no oil in the combustion chamber. at least not too much. what about unburnt fuel smell?

most probably they are connected in some way. you need a closed system to have vacuum. even removing the oil cap will kill most of the vacuum lines in my car. Maybe having a vacuum leak on this one caused something on another one. See if you can find the PCV hose and check that one for lots and lots of oil. on some cars you can look under the AIR filter for oil deposits.
